Group Pricing

Employer funded group of 5 or more employees:
$75 per employee per month.

House Calls

$100 if within 15 miles of Glacier DPC
$200 if within 16-30 miles of Glacier DPC
For other circumstances, please call.

Out of town guests and visiting relatives may be seen on an urgent care basis as a benefit to the member.

*The cost of this visit will be equal to the member’s monthly fee. Additional procedure fees may apply. Members will sign a patient agreement and pay monthly.
